USD 343 Student Medication Forms

We have stock meds at the school now. If you would like your student to be able to come to office to get any of the stock meds listed on the form, please fill out and sign.

Self Carry: This does NOT include OTC medications, we have stock meds at office for students.

Contract will be completed with Nurse at start of school year. This allows for nurse to know that student understands all policies around utilizing medications.
