Test To Stay Consent Form
In the link below you will find the consent form for the Test to Stay screening program. This form must be completed and signed by the parent/guardian prior to student testing. This program is NOT a requirement for students however, it is an option available for those who want to remain in school and to participate in extracurricular activities during quarantine.
USD 343 Test To Stay Consent Form
The USD 343 COVID-19 testing clinic is located at the District Office (205 W. Bridge Street). The clinic is available to all USD 343 students and staff. Testing appointments are available from 7:30 am. to 8:15 am. We ask that you schedule your appointment by contacting the district office the day before taking a COVID-19 test at 785-597-5138 or email jblosser@usd343.org. Test results are usually available in 15 minutes for antigen tests and 30 minutes for PCR tests.
