Enrollment Information
- Actual Birth Certificate (If enrolling for the first time)
- Record of Immunizations
- If out of district student, print off and complete out of district request. If unsure, please provide address from utility statement and we will check for you.
- Other items that will be beneficial to the registration process are:
1. Students Social Security Number
2. Last Report Card
3. Copy of current I.E.P. (Individualized Education Plan) for Special Education. - Any School Records from previous School
- A copy or original of a valid Physical Exam for students interested in playing sports.
- Any legal documentation stating Guardianship, Custody, Restraining Orders, etc.
